What are approriate/useful feedback questions?

Richard Iarossi had a featured blog yesterday on giving showing feedback.  The comments were thought provoking and made me focus on the automatic feedback form that my MLS sends to us if we schedule showings online.   I don't find the questions to be particularly relevant, but my broker has asked that we fill them out, so I do.  There are 6 questions that are multiple choice with a yes/no/maybe or a scale of 1 to 5 and a couple of comments field -- one for the showing agent and one that the owner sees.

MLS feedback form:

1.  How did the buyer like the property?
          (not Interested, interested, very interested, etc)

2.  Would the buyer consider making an offer?
          (yes/no/maybe)

3.  How would you rate the interior of the home?
          (below average, average, excellent, etc)

4.  How would you rate the exterior of the home?
          (below average, average, excellent, etc)

5.  What did the buyer think of the price?
          (overpriced, underpriced, market price, etc)

6.  Are they considering a second showing?
          (yes/no/maybe)

7.  Comments for seller.
          (free format)

8.  Private comments for listing agent.
          (free format)

Based on some people's comments yesterday, I think many of the questions, if answered truthfully, might be revealing buyer's confidential information.  Probably from a software developers standpoint, these are reasonable questions, but at best they don't provide useful information and at the worst might damage a buyer's negotiating position.

I have reached the point where I skip the multiple choice questions and just make private comments to the listing agent about whether the house showed well (whether the seller made an effort to have the house clean and tidy and were the light on, etc.) 

What feedback questions do you think are appropriate and useful?

On my feedback forms in addition to a couple of the ones on your MLS form, I ask for agents' opinions about the landscaping, and whether staging would help, what improvements, if any, should be made to the interior to improve showings, etc.  These questions permit the agents to give input on either what their clients have said or simply advice from the agents.

I'm going to do a drawing to entice agents to preview a listing so I came here to get an idea of a few questions to ask. Thanks to both of you for sharing.
